As a Care Leaver Support Specialist, you will play a critical role in facilitating this support, working closely with key stakeholders to ensure that young people receive the help they need to thrive. Your expertise and passion for working with young people will be invaluable in helping us achieve our goal of empowering care leavers to reach their full potential.

Key Responsibilities

• Provide support and referrals to specialist services in areas such as counselling, accommodation, employment, and education.
• Work closely with young people and their support networks to develop and implement personalized transition plans.
• Liaise with government and non-government agencies to ensure that young people receive the support they need.

About You

• You have a passion for working with young people and delivering meaningful outcomes.
• A tertiary qualification in Youth Work, Social Work, or a related field is essential.
• Experience working with young people who have experienced out-of-home care is highly desirable.
