Leadership Role in Sub-Acute and Community Care

This is a challenging and rewarding leadership role that requires the professional to play a pivotal role in developing, delivering, and evaluating health services at the community level.
- Improve continuity of care for rural communities by strengthening integration across health services, private providers, and community organisations.
- Support a place-based approach to care delivery through advanced clinical competencies.
The successful candidate will have strong leadership and communication skills, with the ability to work effectively in a fast-paced environment. They will be responsible for:
- Developing and implementing plans to improve health outcomes for patients through evidence-based practice and quality improvement initiatives.
- Mentoring and supporting junior staff members to enhance their skills and knowledge.
- Collaborating with other healthcare professionals to achieve common goals and improve patient care.
