Job Description:

This role involves providing evidence-based clinical care in the community, with a focus on Home Care Package, Commonwealth Home Support Program, and Hospital Substitution programs. You will work closely with our clients, their families, and other healthcare professionals to develop and implement personalized care plans that support their unique needs and goals.

  • Deliver targeted occupational therapy to clients with complex health conditions or age-related care needs
  • Prescribe aids, equipment, and home modifications to support client goals
  • Collaborate in a multidisciplinary team for optimal client outcomes
Required Skills and Qualifications:
  • Ongoing registration with AHPRA
  • Valid driver's licence, insured vehicle
