Key Responsibilities

- Conduct thorough client care assessments and develop comprehensive care plans
- Manage a caseload of clients and ensure ongoing compliance with care requirements
- Coordinate and deliver personalised care aligned with client needs, care plans, and allocated resources
- Perform general administrative tasks related to the role

Requirements:

- Relevant qualifications (Cert IV or Diploma in Nursing highly regarded)
- Experience in aged care, NDIS, or mental health case management (preferred)
- Knowledge of My Aged Care, Home Care Packages, and NDIS frameworks
- Proven experience in assessing and planning community-based care
- Proficiency in Microsoft Office 365, SharePoint, Outlook, and rostering software
- Excellent verbal and written communication skills with strong customer service abilities
- Ability to handle incoming calls professionally and courteously
- Strong organisational, problem-solving, and decision-making skills
- Collaborative team player with a proactive attitude
- Ability to build strong relationships with external organisations for client referrals
- Commitment to confidentiality and privacy in line with company policies
