Physiotherapist - Transitional Aged Care Program

We are seeking a skilled and energetic Physiotherapist to join our dynamic team in the Transitional Aged Care Program. As a Physiotherapist, you will work in a multidisciplinary team to provide physiotherapy assessment and treatment for clients in the community.
This role involves case management of clients as they progress through the 8-week program, providing clinical supervision to junior physiotherapists and/or allied health assistants, and contributing to quality improvement projects within the Transitional Aged Care Team.
You will also be involved in liaising with community services/groups and health professionals to assist clients to achieve their TACP goals, and contributing to the education of the Community Aged Care Team and other community teams as required.
Key Responsibilities- Provide physiotherapy assessment and treatment for clients in the community
- Case manage clients as they progress through the 8-week program
- Provide clinical supervision to junior physiotherapists and/or allied health assistants
- Contribute to quality improvement projects within the Transitional Aged Care Team
- Liaise with community services/groups and health professionals to assist clients to achieve their TACP goals
- Contribute to the education of the Community Aged Care Team and other community teams as required
- Tertiary Qualification in Physiotherapy
- Current registration with the Australian Health Practitioners Regulation Agency as a Physiotherapist
- Current NSW drivers' license Class C or above
- Demonstrated ability to work autonomously and as part of a multidisciplinary team
- Demonstrated effective communication, consultation, and interpersonal skills
- Demonstrated commitment to improving clinical practice and service delivery through leadership of quality improvement activities and ongoing professional development
