Compassionate Care Manager Sought

We are seeking a highly skilled and compassionate care manager to join our team in South Australia. As a key member of our client services department, you will be responsible for managing relationships with clients and overseeing service compliance for a portfolio of individuals who have chosen us to fully manage their Home Care Packages.
About the Role- We require an AHPRA registered professional (Nurse, Occupational Therapist or Social Worker) who is passionate about delivering high-quality care and support to vulnerable adults.
- Your primary responsibilities will include completing client assessments and care plans, mitigating client challenges, and facilitating a smooth home care experience.
In this role, you will have the opportunity to make a real difference in the lives of our clients and their families, providing guidance and support that enables them to maintain their independence and dignity.
Key Responsibilities- Client Assessments and Care Planning: Conduct thorough assessments of client needs and develop personalized care plans that meet their unique requirements.
- Relationship Management: Build and maintain strong relationships with clients, their families, and other stakeholders to ensure seamless delivery of care services.
- Care Delivery Oversight: Supervise the delivery of care services, ensuring that they are provided in a safe and effective manner.
- AHPRA registration as a Nurse, Occupational Therapist, or Social Worker.
- Proven experience in care management or a related field.
