As a key member of our team, you will be responsible for providing high-quality medical care to our clients, working collaboratively with our nursing and clinical staff, and contributing to the development of our holistic model of care.
Key Responsibilities- Provide primary medical care to clients, including routine check-ups, health assessments, and treatment of acute and chronic conditions.
- Work collaboratively with our nursing and clinical staff to ensure seamless care coordination and effective communication.
- Contribute to the development and implementation of our holistic model of care, which prioritizes the physical, emotional, and social wellbeing of our clients.
- Participate in regular case conferencing and team meetings to ensure quality outcomes and shared learning.
- Engage with our clients and their families to promote health education, prevention, and early intervention.
