About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Chorus. As a Support Worker, you will play a vital role in connecting our customers with their local team, ensuring they receive the support and care they need to live their lives to the fullest.

Key Responsibilities
  • Provide domestic assistance services to customers, including household chores and personal care
  • Work collaboratively with customers, their families, carers, and other professionals to develop and implement personalized support plans
  • Assist customers with daily living tasks, such as meal preparation, grocery shopping, and transportation
  • Provide social support and companionship to customers, helping to reduce feelings of loneliness and isolation
  • Participate in team meetings and contribute to the development of new ideas and initiatives
  • Maintain accurate records and reports, ensuring compliance with organizational policies and procedures
Requirements
  • Previous experience working with seniors and individuals with disabilities is highly desirable
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with customers and colleagues
  • Ability to work independently and as part of a team, with a strong focus on customer-centered care
  • Current NDIS Worker Screening Check and Western Australian Drivers Licence
  • Ability to work flexible hours, including evenings and weekends
