About the Role

Grafton Community & Allied Health is seeking a Social Worker to join their Specialist Palliative Care team. This role works as part of a multidisciplinary team providing inpatient and community social work services to clients, families, and carers of the specialist palliative care service.

This role will provide therapeutic services to clients and provide specialist consultation and support to other team members in the management of complex cases.

You will enjoy working within the model of palliative care to identify and address psychosocial needs.
