The Social Work Department operates with a focus on quality of life and participation in care. A primary goal is to increase the quality of life and participation in care for patients and their families.
Social Work Role- Identify and address psychosocial impacts of illness and hospitalization by providing services that enhance the patient experience.
- Assist patients and their families with a smooth transition from hospital to home, ensuring continuity of care.
The role involves working in a multi-disciplinary team environment, providing advanced social work services including:
- Liaising with health professionals, family members, and community agencies to facilitate effective care.
- Conducting comprehensive assessments through clinical reasoning to develop timely discharge plans.
- Consulting with colleagues and staff from other disciplines on issues relevant to patient care, and providing expertise in casework management.
- Maintaining a complex caseload within the clinical unit and social work department.
Required Skills and Qualifications:
- Eligibility for membership of the Australian Association of Social Workers.
- Bachelor of Social Work or recognized equivalent qualification.
- Experience in an acute hospital setting is desirable.
- Ability to work effectively in a team and with families, demonstrating initiative and facilitative qualities.
