DPV Health is seeking an experienced Senior Care Manager to join our team, where you will play a vital role in delivering high-quality care to our clients. As a Senior Care Manager, you will work collaboratively with our multidisciplinary team to develop and implement care plans that cater to the unique needs of each client.

  • Conduct comprehensive assessments of clients' needs, develop and implement personalized care plans, and coordinate with healthcare professionals to ensure seamless care delivery.
  • Provide guidance and support to junior care managers, ensuring they have the necessary skills and knowledge to deliver exceptional care.
  • Collaborate with our Allied Health Services, Social Support, Property, and Fleet teams to ensure a holistic approach to care delivery.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes for our clients.
  • Develop and maintain strong relationships with clients, their families, and caregivers, ensuring they feel supported and empowered throughout their care journey.

What We're Looking For

  • A Bachelor's degree in Nursing or a related field.
  • Extensive experience in case management, with a strong understanding of community aged care services and consumer-directed care.
  • Proven ability to work with ACAT, RAS, Care Navigators, and other referrers to ensure seamless care coordination.
  • Strong budget management skills, with the ability to manage complex client budgets.
  • High-level problem-solving skills, with the ability to think critically and analytically.
