The role of the Clinical Dietitian in our Community Rehabilitation Program is to provide expert dietetic care to clients, utilizing a person-centered approach. This involves conducting thorough assessments, developing personalized intervention plans, and providing ongoing education and support to ensure optimal health outcomes.

Key Responsibilities:

  • Provide high-quality dietetic assessment, treatment, and patient management services to clients within the Community Rehabilitation Program">
  • Work collaboratively with the multidisciplinary team to achieve program goals and objectives">
  • Contribute to the development and implementation of service models that meet the needs and demands of the community">
  • Participate in regular clinical supervision and quality improvement activities to ensure best practice and continuous improvement">

Requirements:

  • Current registration as a practicing dietitian with Dietitians Australia and completion of the APD program">
  • Ability to competently perform high-quality dietetic assessment, treatment, and patient management, including thorough and appropriate discharge planning">
  • Well-developed interpersonal and communication skills">
  • Demonstrated ability and experience working within a multidisciplinary team environment">
  • Commitment to continuous and innovative improvement through quality and research activities">
  • Current Victorian Driver's license">

  • Experience working in community rehabilitation or similar settings">
