About the Role

We are seeking a highly motivated and compassionate Disability Support Professional to join our team at Endeavour Foundation. As a Support Worker - Mental Health, you will play a vital role in supporting individuals with disabilities to live their best lives and achieve their goals.

Key Responsibilities
  • Provide support and assistance to individuals with disabilities in their homes and in the community, promoting independence and self-advocacy.
  • Develop and implement individualized support plans to meet the unique needs and goals of each person.
  • Assist with daily living tasks, such as personal care, meal planning, and light cleaning, while promoting a safe and healthy environment.
  • Facilitate community connections and social activities, such as outings and group events, to enhance the quality of life for individuals supported.
  • Collaborate with other support workers, healthcare professionals, and family members to ensure comprehensive and coordinated care.
Requirements
  • Experience working in disability services, preferably in a Supported Independent Living (SIL) environment.
  • Strong communication and interpersonal skills, with the ability to build rapport with individuals, families, and colleagues.
  • Flexibility to work various shifts, including days, evenings, and weekends, with a minimum of 2 sleepover shifts per fortnight.
  • Ability to meet the physical demands of the role, including manual handling tasks.
  • Commitment to maintaining confidentiality and upholding the principles of person-centered care.
  • Good computer skills and experience using software applications, such as Word, Excel, and Outlook.
