Job Description

Our mission is to empower children in Out Of Home Care (OOHC) to reach their full potential. We provide support to their carers and birth parents, ensuring a safe environment for these children to thrive.

This role presents an opportunity to case manage children and young people in OOHC, supporting them, their carers, and birth families to achieve the best possible outcomes.

  • We work with a range of clients, tailoring our approach to meet the unique needs of each individual.
  • Engage with clients using appropriate assessment and risk mitigation strategies.
  • Develop and implement comprehensive plans, ensuring active involvement from children and young people.
  • Foster transparent communication by openly sharing client plans with relevant agencies.
  • Cultivate strong relationships with service providers to deliver holistic support.
Key ResponsibilitiesClient Assessment and Planning

Conduct thorough assessments to develop, implement, and review client plans, ensuring active participation from children and young people.

Communication and Collaboration

Facilitate open communication by sharing client plans with clients and relevant agencies, and coordinate with various service providers to ensure comprehensive support delivery.

Culturally Safe Practice

Work holistically to ensure the home and support network contribute to development, improve long-term outlooks while maintaining connections with birth families and culture.
