
Community Care Specialist

As a Part-Time Intake Clinician, you will play a vital role within our Intake Team, supporting clients accessing a diverse range of community health services.
About the Role:
- Initiate intake processes and create episodes of care by establishing strong relationships with clients and their families.
- Schedule appointments and maintain accurate client data and documentation using digital systems and excellent organisational skills.
- Work collaboratively across disciplines with a team-oriented approach, fostering a culture of mutual respect and open communication.
- Utilise your expertise in digital systems and general computer skills to deliver efficient and effective work practices.
Key Responsibilities:
- Provide high-quality client care and support, prioritising their needs and promoting positive outcomes.
- Develop and maintain effective working relationships with colleagues, healthcare professionals, and other stakeholders.
- Contribute to the development and implementation of strategies to improve client engagement and satisfaction.
- Participate in quality improvement initiatives, identifying areas for growth and implementing changes to enhance service delivery.
Requirements:
- Relevant qualifications as a Nurse, Allied Health Assistant, or Allied Health Professional, with a strong focus on person-directed care.
- Experience in delivering person-centred care within a community setting, with a proven ability to build trust and rapport with clients and their families.
- Strong interpersonal and communication skills, both written and verbal, with the ability to adapt to diverse situations and individuals.
- Excellent organisation and time management skills, with the ability to prioritise tasks and meet deadlines.
