Palliative Care Specialist Social Worker

**Job Overview**
We are seeking a highly skilled Palliative Care Specialist Social Worker to join our team. As a key member of our interdisciplinary team, you will be responsible for providing specialist social work services to patients and their families in palliative care.
**Key Responsibilities:**
- Conduct psychosocial assessments and develop individualized care plans to meet the needs of patients and their families.
- Provide counseling and support to patients and their families to cope with grief, loss, and bereavement.
- Collaborate with other healthcare professionals to ensure seamless delivery of patient care.
- Develop and implement quality improvement initiatives to enhance patient outcomes and experiences.
**Requirements:**
- Degree in Social Work or equivalent qualification.
- Postgraduate clinical experience in psychosocial assessment and care planning.
- Excellent communication and interpersonal skills.
- Able to work effectively within a multidisciplinary team.
