About the Role
We are seeking an experienced Occupational Therapist to join our multidisciplinary team at Sunbury and Cobaw Community Health. This is a part-time or full-time opportunity that offers flexible days and hours, allowing you to choose a schedule that suits your needs.
Job Description:
The successful candidate will be responsible for providing occupational therapy services to clients across a variety of age groups, working with a range of funding bodies, including the National Disability Insurance Scheme (NDIS), Home Care Packages, and the Commonwealth Home Support Program.
You will have the opportunity to work in a dynamic community health setting, collaborating with a large and experienced team of OTs, and contributing to specialist clinical programs, such as cardiac and pulmonary rehabilitation, falls prevention, and pain management.
Required Skills and Qualifications:
- At least two years of clinical occupational therapy experience
- Excellent interpersonal skills and the ability to communicate effectively with clients, families, and carers
- Ability to complete goal-directed care planning, case coordination, equipment and home modification prescription, and chronic illness management
- High level of written and verbal communication and interpersonal skills to operate in a collaborative manner within the organisation
- Knowledge and experience working with My Aged Care, State Wide Equipment Program, and Geat 2 Go
- Knowledge and experience working under fee-for-service models such as the NDIS and/or Home Care Packages

Estimated Salary:
