We are seeking a dedicated and compassionate Disability Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in ensuring that our patients receive high-quality care and support.
About the Role:
This is an exciting opportunity for a motivated individual who is passionate about delivering exceptional patient care. Your primary responsibility will be to coordinate care for patients, including scheduling appointments and arranging for diagnostic tests. You will work closely with healthcare providers to ensure timely follow-up and coordination of care. Additionally, you will educate patients and families on healthcare services and treatment plans, providing information and referrals to community resources as needed.
Responsibilities:

- Advocate for clients' rights and needs, addressing any issues that may arise
Requirements:
To be successful in this role, you will require a minimum of 2 years of experience in a healthcare setting. You must possess excellent communication and interpersonal skills, as well as strong organizational skills and attention to detail. Proficiency in Microsoft Office and electronic medical record systems is also essential. A strong ability to work independently and as part of a team is required, as well as an understanding of relevant laws and regulations.
What We Offer:
In addition to a competitive hourly rate of $28.00 - $38.00 per hour, we offer flexible scheduling and a supportive work environment. This is a casual position, with opportunities for professional growth and development.
