The Disability Support Coordinator is a vital position that plays a key part in empowering individuals with disabilities to achieve their goals and increase their independence through community connections.

About You

">
  • ">
  • Certificate IV in Mental Health or an equivalent qualification; or relevant practical experience required.">
  • A commitment to child, youth, and disability safety is essential.">
  • Excellent time management, administration, and delegation skills are necessary for maintaining accurate records and reports.">
  • Demonstrated ability to engage with diverse backgrounds and cultures to explain NDIS processes in plain English.">
  • Strong attention to detail and proficiency in Microsoft Office Suite and client information management systems are required.">
  • A current NDIS Worker Screening Check and Working with Children's Check are necessary.">
  • Ability to work independently, prioritize tasks, and manage deadlines effectively.">

Key Responsibilities

  1. ">
  2. SUPPORT PARTICIPANTS WITH COORDINATION OF NDIS PLANS, NEGOTIATING APPROPRIATE SERVICES AND CONNECTING WITH COMMUNITY SUPPORT AND MAINSTREAM SERVICES.">
  3. PROMOTE A CLIENT-CENTRED APPROACH TO SERVICE PROVISION BY EFFECTIVELY PROVIDING RELEVANT INFORMATION TO MAXIMISE PARTICIPANTS' LOCUS OF CONTROL AND ABILITY TO CHOOSE SERVICES.">
  4. NAVIGATE, INFLUENCE, AND NEGOTIATE WITH PUBLIC SECTOR AND COMMUNITY-BASED SERVICE SYSTEMS FOR DELIVERY OF APPROPRIATE SUPPORTS IN ACCORDANCE WITH NDIS PLANS.">
  5. ENSURE CONTINUOUS DEVELOPMENT OF PROFESSIONAL NETWORK TO MEET PARTICIPANT NEEDS AND PROVIDE BEST QUALITY CARE IN CONSULTATION WITH MULTIDISCIPLINARY TEAM FROM ADS AND OTHER ALLIED HEALTH SERVICES.">
  6. FOLLOW UP AND PREPARE FOR NDIA PLAN MEETINGS AND HELP PARTICIPANTS NEGOTIATE THEIR PLANS ACCORDING TO THEIR GOALS AND NEEDS.">
  7. ENSURE PARTICIPANTS' INTERVENTION PLANS ARE DEVELOPED AND IMPLEMENTED WITHIN 2 WEEKS OF REFERRAL.">
  8. BUILD NDIS PARTICIPANT AND SUPPORT NETWORK CAPACITY TO UNDERSTAND AND NAVIGATE SERVICE SYSTEMS.">
  9. COMPLETE DISCHARGE PLANNING BY CONSULTING WITH PHYSICIANS, NURSES, SOCIAL WORKERS, AND OTHER HEALTHCARE WORKERS; CONTRIBUTING TO PARTICIPANT CARE CONFERENCES.">
  10. PROVIDE EXPERT ADVICE AND CONSULTATION TO NDIS PARTICIPANTS AND THEIR FAMILIES ON THE CHANGING NDIS ENVIRONMENT.">
  11. SUPPORT NDIS PARTICIPANTS, FAMILIES, AND CARERS TO DEVELOP RESILIENCE IN THE NDIS PARTICIPANTS' NETWORK.">
  12. SUPPORT PARTICIPANTS IN THE REVIEW OF THEIR PLANS INCLUDING ANNUAL REVIEWS, CHANGE OF CIRCUMSTANCES OR NEW PLANS THAT HAVE INSUFFICIENT SUPPORT.">
