About the Role

This is an exciting opportunity to work with our team as a practitioner in the Mapping, Education and Carers Service. We are looking for a motivated and enthusiastic individual who is passionate about improving care for people with dementia and their carers.

The service covers Gwent and the post holder will be expected to travel across different sites. The role involves working in diverse settings, including care homes and in-patient hospital settings.

The successful candidate will receive supervision and opportunities to explore ongoing professional development. English and/or Welsh speakers are equally welcome to apply.

We support flexible working arrangements.

Main Responsibilities
  • Dementia Care Mapping: Observing people with dementia within care environments, assessing needs and supporting teams in providing care.
  • Carers Education: Direct support and education for carers of loved ones with dementia.
  • Staff training: Designing and delivering staff learning and development.

  • Relevant clinical skills

  • Previous relevant clinical experience.
  • Willingness to develop professional development to fulfill the role.
