CDM Nurse

We are seeking a skilled and experienced CDM Nurse to join our team at ForHealth Medical Centres in Mt. Druitt. As a CDM Nurse, you will play a vital role in supporting our patients to manage their chronic conditions and improve their overall health.
About the RoleThis is a part-time opportunity that requires a strong passion for preventative healthcare and a commitment to delivering high-quality patient care. As a CDM Nurse, you will work closely with our General Practitioners to identify and manage chronic diseases, develop care plans, and empower patients to take ownership of their health.
Key Responsibilities- Develop and implement care plans to support patients with chronic diseases
- Conduct health assessments and provide coaching on health behaviour change principles
- Work collaboratively with our multidisciplinary team to ensure seamless patient care
- Manage administrative tasks related to CDM appointments
- Monitor patient progress and adjust care plans as needed
- AHPRA registration as an Enrolled or Registered Nurse
- Proven experience in Chronic Disease Management and health assessments
- Strong organisational and time management skills
- Excellent communication and customer service skills
- A passion for delivering high-quality patient care
