Role Overview:

As a dedicated Care Manager, you will play a vital role in supporting older Australians to maintain their independence and live safely in their own homes.

With a strong focus on delivering exceptional client care, you will lead the way in providing high-quality homecare services that meet the unique needs of each individual.

Working collaboratively with our experienced team, you will be responsible for managing client service delivery across all levels under government-funded programs, including CHSP and Homecare Packages.

You will engage with internal teams to facilitate seamless client onboarding, while proactively engaging in continuous improvement across practice and service delivery.

">Key Responsibilities:">

- Deliver excellent client care for elderly Australians
- Manage and oversee a high standard of care provided by homecare providers and staff
- Develop, monitor and reassess care plans in consultation with clients, their families and other healthcare professionals.
- Work in collaboration with our Nurses, Regional and other Managers to facilitate, develop and shape our service offerings. Critically, you'll ensure our services continue to be of the highest standard.
- Monitor the budget of care packages and ensure clients understand service fees.
- Engage with internal teams to facilitate client onboarding through the lead to conversion process.
- Utilise your influencing and partnering skills work collaboratively with our schedulers, clinical team and support staff.
- Proactively engage in continuous improvement across practice and service delivery.

">Requirements:">

- A Certificate III in Individual Support or equivalent qualification
- Experience in aged care, with knowledge of Home Care Packages, Commonwealth Home Support Program and Aged Care Quality Standards is desired
- Current First Aid Certificate (HLTAID003/4) and current CPR certificate (HLTAID001) or willingness to obtain (organisation will organise at no cost)
- The right to work in Australia
- A current driver's licence
- National Police Clearance or willingness to obtain (organisation will organise via Workpro at no cost)
