Aboriginal Care Management Role

This position involves providing service brokerage and individualized supports to clients and their families to increase their social and emotional wellbeing.

The role will work with people in community and residential programs who may be experiencing difficulties with alcohol and other drugs (AOD), chronic disease, mental health illnesses, domestic and family violence, housing, and other health and social issues.

  • Job Responsibilities:
  • Provide service brokerage and individualized supports to clients and their families.
  • Work with people in community and residential programs who may be experiencing difficulties with AOD, chronic disease, mental health illnesses, domestic and family violence, housing, and other health and social issues.
  • Communicate effectively with service stakeholders, both verbally and in writing.
Requirements:
  • Must be of Australian Aboriginal descent and identify as an Aboriginal person by the local Aboriginal community.
  • Current Certificate IV in Alcohol and Other Drugs or related field such as Mental Health, Aboriginal Health, Community Services, Allied Health Assistance, or Social Care.
  • Demonstrated knowledge and understanding of the social and emotional wellbeing needs and issues of Aboriginal people.
  • Proven experience in working with Aboriginal clients and their families and managing a caseload.
  • Current unrestricted NT Drivers Licence.
