Community Health Navigator

We are seeking a motivated and compassionate Community Health Navigator to join our team at Anyinginyi Health Aboriginal Corporation.
The successful candidate will be responsible for coordinating the development and implementation of health programs and services in the Barkly Region, with a focus on improving the health and wellbeing of Aboriginal children and families.
Main Responsibilities:
- Establish and maintain relationships with community members, healthcare providers, and other stakeholders to ensure effective program delivery.
- Develop and implement health education programs, including workshops, training sessions, and one-on-one support, to promote healthy lifestyles and disease prevention.
- Conduct home visits and provide individualized support to families to address their health needs and concerns.
- Collaborate with healthcare providers to ensure seamless referrals and care coordination for program participants.
- Collect and analyze data to monitor program effectiveness and identify areas for improvement.
Requirements:
- Postgraduate degree in a health-related field, or equivalent experience.
- Strong knowledge of indigenous health issues, cultural protocols, and community development principles.
- Excellent communication, interpersonal, and project management skills.
- Ability to work in a fast-paced environment, prioritize tasks, and meet deadlines.
- Valid Ochre Card (NT Working with Children's) card and National Police Clearance (obtained prior to commencement).
