Job DescriptionIn this role, you will be responsible for providing high-quality care and support to clients living in the community. You will work with clients to develop personalized care plans that meet their unique needs and preferences. This will involve assisting with daily living tasks such as showering, dressing, and meal preparation, as well as supporting clients to engage in recreational and social activities.
You will also work closely with other healthcare professionals to ensure that clients receive comprehensive and coordinated care. As a Supported Living Facilitator, you will have the opportunity to make a real difference in the lives of your clients and help them to live independently and with dignity.
Required Skills and QualificationsTo be successful in this role, you will need to have excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment. You will also need to have a positive attitude, a willingness to learn, and a commitment to delivering high-quality care and support.
A current NDIS Worker Screening Check and current CPR and First Aid Certificate are also essential requirements for this role. In addition, you will need to have a valid driver's license and access to a reliable vehicle, as well as car insurance in your name.
