About the Role

We are seeking a highly motivated Registered Nurse to join our team as a Care Manager. As a key member of our care coordination team, you will be responsible for providing clinical guidance and oversight to ensure the delivery of high-quality care to our clients.

Responsibilities
  • Provide clinical guidance and support to our care coordination team
  • Manage client cases and provide ongoing care planning and coordination
  • Develop and maintain relationships with clients, families, and healthcare professionals
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Contribute to quality improvement initiatives and service development
Requirements
  • Registered Nurse with current AHPRA registration
  • Minimum 3 years' experience in patient/client care management
  • Strong organisational and time management skills
  • Excellent written and verbal communication skills
  • Able to work effectively in a team environment
