
Disability Care Planner

The Disability Care Planner is responsible for supporting children with complex disabilities who require additional mainstream supports to sustain their care in the family home or residing outside.
This role involves working closely with families, service providers and other stakeholders to develop and implement individualized care plans that meet the unique needs of each child.
Main Responsibilities- Support and Facilitation
- Provide support to families and caregivers to ensure they have the necessary resources and information to care for children with complex disabilities.
- Facilitate communication and collaboration between families, service providers and other stakeholders to ensure seamless delivery of care services.
- Care Planning and Coordination
- Develop and implement individualized care plans that address the unique needs of each child.
- Coordinate with service providers and other stakeholders to ensure timely and effective delivery of care services.
- Case Management
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
- Work with families and service providers to identify and address any barriers to care.
This role requires a high degree of emotional intelligence, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment.
The ideal candidate will possess a strong understanding of disability support principles and practices, as well as experience working with children and families.
A relevant qualification in a field such as social work, psychology or education would be highly regarded.
