Therapy Solutions Consultant
We are seeking a Therapy Solutions Consultant to work with individuals and communities to develop and implement rehabilitation plans that promote independence and quality of life.
Key Responsibilities
- Conduct assessments and develop personalized therapy plans for clients
- Provide case management and support services to clients in their homes, clinics, and community settings
- Collaborate with multidisciplinary teams, including mental health professionals, social workers, and healthcare providers
- Educate clients and their families about resources and services available to support long-term care and recovery
Requirements
- 5+ years of experience in mental health case management and therapy services
- Current registration as a Registered Nurse (Clinical Nurse / Clinical Nurse Consultant) or equivalent qualification
- Ability to work autonomously in regional & remote settings and travel as required
- Passion for delivering high-quality mental health care and making a positive impact on diverse communities
