Aboriginal Health Worker Job Opportunity

This is a dynamic and impactful role within the field of First Nations health. As an Aboriginal Health Worker, you will play a pivotal part in supporting chronic disease management and empowering your community with culturally safe care.

The successful candidate will be responsible for assisting clinicians with client intake, assessments, follow-ups, and discharge processes to ensure smooth and seamless care.

  1. You will lead group sessions, delivering culturally safe and engaging health education and yarning circles.
  2. Maintain accurate client electronic medical records when appropriate.
  3. Help First Nations people access health services and support them through the process.
  4. Assist clinicians with completing assessments and surveys to evaluate health outcomes.
  5. Coordinate and facilitate telehealth sessions between clients and clinicians.
  6. Work with the Primary Health team to identify and remove barriers preventing First Nations individuals and families from accessing health services.
  7. Provide culturally appropriate education to improve health literacy within the community and empower individuals to make informed health decisions.

To be successful in this role, you must possess:

  • Cultural Connection: You identify as Aboriginal and/or Torres Strait Islander.
  • Strong Communication Skills: You have excellent interpersonal skills, with the ability to tailor your approach to meet the unique cultural needs of the community.
  • Relevant Qualifications: You've completed or are willing to complete a minimum Certificate 3 qualification in Aboriginal Health, Community, or Disability.
  • Commitment to Growth: You're passionate about learning new skills and following through on tasks with dedication and attention to detail.
  • Community Engagement: You have a natural ability to build strong relationships and connect with Aboriginal people in your community.
  • Health Knowledge: You understand how to identify and support First Nations community members with chronic health issues and guide them to the right referral pathways.
