Key Position Description

 

We are seeking an experienced professional to fill a role in delivering culturally safe three-year flexible support packages to participants who need support to sustain tenancies or move into stable housing.

 

Key Responsibilities:
  • Delivering case management and service coordination activities while liaising with other agencies, community resources, and services.
  • Building strong relationships with participants who have experienced chronic homelessness.
  • Supporting participants to move and settle into their tenancies and linking them with their local communities.
  • Working long-term with a small caseload to focus on individualised goal planning.
  • Performing regular outreach appointments, focusing on preventative support and strategies that help break the cycle of homelessness and achieve participant outcomes.

 

Required Skills and Qualifications:
  • An appropriate degree qualification relevant to community services.
  • Experience in case management and navigating complex service systems.
  • A passion for sustaining tenancies and ending homelessness in Melbourne.
  • The ability to adapt to participants' needs effectively and include the voice of the participant while collaborating with different services.
  • The ability to demonstrate de-escalation and effective response tactics towards challenging behaviour and crisis situations.
