Healthcare Professional for Patient Care Coordination

We are seeking a dedicated healthcare professional to join our team as a Care Manager. As a key member of our interdisciplinary care team, you will collaborate closely with primary care clinicians and other healthcare providers to deliver comprehensive and coordinated patient care.
The ideal candidate will possess strong organizational and communication skills, with the ability to work effectively in a fast-paced environment. A keen attention to detail and a passion for delivering exceptional patient outcomes are essential for this role.
Key Responsibilities:
- Develop and implement personalized care plans that cater to the unique needs and goals of each client.
- Serve as the primary point of contact for clients and their families, providing regular updates on care plans and addressing any concerns or questions they may have.
- Collaborate with healthcare providers, social workers, and other stakeholders to ensure seamless coordination of care and optimal patient outcomes.
Requirements:
- A degree in nursing from an accredited institution is required.
- A valid nursing license (RN or LPN) is necessary for this position.
- Previous experience in caring for patients with chronic diseases is highly desirable.
