This is an exciting opportunity to join our dynamic multidisciplinary team as a Community Mental Health Nurse Specialist.


Job Description:

We are seeking an experienced and skilled Registered Nurse to work within our Assertive Outreach Team, providing care coordinated services to consumers diagnosed with a severe and enduring mental illness.


The successful applicant will be responsible for the provision of patient-centred; evidence-informed nursing care that promotes identified health outcomes. This includes assessing, planning, implementing, and evaluating nursing care in collaboration with individuals and the multidisciplinary healthcare team.


Required Skills and Qualifications:
  • Current nursing registration with the relevant regulatory body
  • Demonstrated understanding of evidence-informed practice and critical thinking
  • Demonstrated commitment to person-centred care and core values
