About This Role

The Community Transitions Clinician plays a crucial role in assisting individuals with severe mental illness to transition from custody to the community. The clinician collaborates with multidisciplinary teams, primary health networks, and other organizations to ensure continuity of care.

Key Responsibilities

Work Environment

The Community Transitions Team is a statewide service that supports people from various correctional centers across Sydney metropolitan and regional NSW. Occasional travel to different locations across NSW may be required.
