Our organization is seeking a skilled Clinical Care Coordinator to provide clinical care coordination and mental health support for clients. The successful candidate will have expertise in case management, risk assessment, and care planning.

The role involves engaging with clients to address their psychosocial needs and improving mental health outcomes. The successful candidate will work closely with internal and external stakeholders to deliver recovery-focused care.

Key responsibilities include:

  • Providing clinical case management, brief interventions, and risk assessments
  • Engaging with clients to address their psychosocial needs and improve mental health outcomes
  • Collaborating with internal and external stakeholders to support recovery-focused care
  • Maintaining accurate client records
  • Participating in clinical supervision and ongoing training

The ideal candidate will possess excellent communication skills, a strong understanding of the mental health sector, and experience working with vulnerable populations. They will also be able to demonstrate a commitment to delivering high-quality services and supporting the growth and development of our team.

Required Skills and Qualifications

The following qualifications are required:

  • Psychologist, Occupational Therapist or Registered Nurse with full registration with AHPRA
  • Social Worker with AASW membership
  • Master's Degree in Counselling and registration with PACFA (Clinical) or ACA (Level 3/4)

The ideal candidate will have:

  • Ability to obtain a Working with Vulnerable People/Children Check
  • Australian Citizenship and eligibility for Baseline Clearance

  • $55 - $65 p/h + super
