About the Role
Home Support Service Planners play a crucial part in enabling individuals to maintain their independence and live comfortably in their own homes. They work closely with customers to develop personalized support plans that cater to their unique needs and goals.
Key Responsibilities:
- Collaborate with customers to design services that align with their objectives and requirements
- Coordinate service delivery to ensure accurate customer schedules
- Build and maintain effective relationships with external stakeholders, including provider partners and government agencies
- Act as the primary point of contact for resolving customer concerns and complaints
- Conduct regular support plan reviews to identify areas for improvement and implement changes
- Monitor and mitigate risks to ensure customers remain healthy and safe at home
- Meet individual performance metrics and contribute to team targets
Required Skills and Qualifications
To excel in this role, you will need:
- Knowledge of ageing-related issues, including dementia-related disorders
- Intermediate/advanced level proficiency in Microsoft Office
- Strong documentation and administration skills, with experience in scheduling a plus
- Excellent interpersonal, communication, and problem-solving skills
