Key Responsibilities
  • Develop and implement individualized support plans to meet the unique needs of participants
  • Build and maintain strong relationships with participants, their families, and service providers
  • Coordinate and facilitate access to funded and community supports
  • Provide advocacy and support to participants to ensure their rights and needs are met
  • Collaborate with internal and external stakeholders to ensure seamless service delivery
Requirements
  • Tertiary qualification in a relevant field (e.g. social work, disability studies, community services)
  • Minimum 2 years' experience working in the disability sector
  • Strong communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Competence in data management and customer management systems
  • Current Australian driver's license
  • Satisfactory completion of safety screening checks
