About the Role

Community Services #1 is seeking a qualified Registered Nurse to join our Home Care team. As a Clinical Care Specialist, you will work closely with our Support at Home Partners to deliver high-quality clinical services to our clients. Your primary responsibility will be to ensure that clients receive the necessary care and support to achieve their goals and maintain their independence.

Key Responsibilities
  • Deliver clinical services to clients in accordance with their Support Plan and the Home Care Package program.
  • Work collaboratively with the Support at Home team to develop and implement client care plans.
  • Contribute to the development of the clients Support Plan in collaboration with the Support at Home Partners.
  • Participate in quality assurance processes to ensure the delivery of high-quality clinical services.
Requirements
  • Registered Nurse with current Registration under the Australian Health Practitioner Regulation Agency (AHPRA).
  • Demonstrated sound understanding of risk assessment and duty of care for home and community care services.
  • Demonstrated experience in successfully creating a culture of continuous program improvement.
  • Demonstrated excellent team management and organisation skills.
  • Demonstrated commitment to a person-centred, enablement approach that upholds the independence, community participation, choice, and rights of people over 65 years and carers.
