The Opportunity

Berry Street is seeking a dedicated and compassionate professional to join our team as a Case ManagerHybrid. In this role, you will work closely with young people in Out of Home Care, providing assistance and support to help them achieve their goals and develop independent living skills.

About the Role

As a Case ManagerHybrid, you will be responsible for building relationships with young people, their carers, and care teams to identify and address their support needs. You will work collaboratively to develop and implement individualized plans, providing guidance and resources to help young people achieve their goals.

Key Responsibilities
  • Provide assistance to young people in Out of Home Care, including support with daily living skills, education, and employment.
  • Work closely with care teams and carers to identify and address the support needs of young people.
  • Develop and implement individualized plans to help young people achieve their goals.
  • Provide guidance and resources to help young people develop independent living skills.
Requirements
  • Bachelor level qualification in Youth Work, Social Work, or associated human service field.
  • Excellent written and oral communication skills, including public speaking, presentations, and facilitation.
  • Comprehensive understanding of the statutory out of home care system and issues related to the support needs of young people.
  • Demonstrated ability to flexibly manage competing priorities and stressful situations, monitoring own stress levels and practicing and promoting self-care strategies.
