The Care Coordinator will be responsible for building strong connections with clients and their families, assessing client needs, conducting Client Care Assessments and Care Planning, maintaining accurate records, supporting clients to meet their goals, and providing feedback to colleagues.

Essential criteria include experience in assessing and planning care needs, a sound understanding of relevant standards, organisational skills, COVID-19 vaccinations, and relevant certifications such as a Registered Nurse or Enrolled Nurse. Ability to obtain national police checks, blue cards, drivers' licences, NDIS worker screening checks, and first aid certificates are also necessary.
