Job Overview

This part time role offers the opportunity to work as a Support Worker in our Home Care services (CHSP, VHC and HCP) team based in Perth Metro South East region. You will be working with a friendly and supportive team, providing individualised care tailored to the needs of each client and their families.

Responsibilities

  • Provide meal assistance, including shopping and meal preparation
  • Transport and accompany clients on shopping trips and appointments
  • Social support & activities, including outings and social activities
  • Personal care, including assistance with personal grooming, hygiene, bathing, dressing
  • Support clients with health conditions, including skin care, medication reminders and diabetes management
  • Assist with mobility
  • Domestic assistance, including cleaning and laundry

Requirements

  • A kind, caring and respectful approach to service provision
  • Certificate III in Aged Care / Individual Support (or similar) or equivalent experience
  • Self-motivated with the ability to work effectively in both a team environment and independently with minimal supervision
  • Ability to work with individuals from a range of social, financial and cultural backgrounds
  • Strong communication and interpersonal skills
  • Current driver's license and reliable car
  • Current national police check
  • Prior experience in a similar role in community/home care (desirable)
