SA Health is seeking a dedicated and skilled Occupational Therapist to join our Memory Support Transition Service Team.

The team will provide support to patients with memory loss and Behavioural and Psychological Symptoms of Dementia (BPSD), aiming to safely transition them from the Acute and Subacute inpatient environment into community accommodation.

About the Role:

As an Occupational Therapist, you will work closely with the Geriatric Evaluation and Management (GEM) Nurse Practitioner Candidate and the Multi-D team to develop patient-centred management plans, supporting patient well-being and transition to new environments.

You will also provide liaison to Residential Care Facilities, integrating care between SA Health and the community setting by providing direct patient care and educating staff to care for patients with memory loss and responsive behaviours in line with patient behaviour support plans.

About You:
  • A degree or other qualification entitling registration as an Occupational Therapist with the Occupational Therapy Board of Australia
  • Experience as a clinician in assessing and providing high-quality Occupational Therapy services to clients with various conditions requiring management of issues associated with cognitive impairment and/or changed behaviours
  • Proven experience in delivering high-quality and safe care consistent with the National Safety and Quality Health Service Standards
  • High-level interpersonal, negotiating, counselling, and problem-solving skills with the ability to communicate effectively with patients, professional staff, and community agencies

Salary: $88,255 - $102,240 per annum (pro rata) plus salary packaging opportunities.
