The Role

We are offering a permanent full-time position (76 hours per fortnight) for a skilled Clinical Pharmacist who will provide comprehensive clinical pharmacy services to support quality use of medicines, deliver education to our multidisciplinary team, and offer expert advice on health promotion, disease prevention, drug selection, and the quality use of medicines.

Responsibilities

  • Conduct medication chart reviews and medication reconciliation to ensure accurate and safe patient care.
  • Dispense and supply medication and pharmacy products with precision and attention to detail.
  • Coordinate patient medication requirements for inpatient stays, day infusions, and discharges, ensuring seamless transitions between care settings.
  • Develop and maintain strong relationships with key stakeholders to deliver exceptional customer service and support patients, carers, hospital staff, and associated employees.
  • Educate and advise staff and customers on best practices in health promotion, disease prevention, drug selection, and the quality use of medicines.

What You'll Need

  • A registered pharmacist with current AHPRA registration.
  • A minimum 1-year experience as a pharmacist in an Australian-based pharmacy, although prior hospital pharmacy experience will be well regarded.
