Home Care Support Worker

We are seeking a dedicated and compassionate Personal Care Professional to join our team in providing exceptional in-home care and support to clients across Western Australia.
The ideal candidate will be passionate about delivering holistic, client-focused care and have excellent communication and interpersonal skills. They will work closely with registered nurses to deliver tailored care plans and assist clients with daily living activities.
Key Responsibilities:
- Provide personal care and assist clients with daily activities, following the client care plan.
- Work collaboratively with the care team to identify client needs and deliver high-quality care.
- Communicate effectively with clients, their families, and team members.
- Support clients with light housekeeping duties to promote independence.
- Maintain accurate daily client notes and report any concerns to the RN/Care Manager.
Qualifications and Skills:
- Certification in Aged Care or Disability is essential.
- Current first aid and manual handling certifications are required.
- A reliable vehicle and driver's license are necessary for travel to client homes.
- Federal Police Clearance is mandatory for this role.
- Excellent communication and interpersonal skills are essential for success in this position.
