We are seeking a highly skilled and experienced Clinical Services Coordinator to join our team at Southern NSW Local Health District. As a key member of our clinical services team, you will be responsible for providing evidence-based clinical services to individuals of all ages referred to our service with issues related to substance misuse and/or dependency.
Key Responsibilities- Develop and implement collaborative, person-centred care plans for consumers affected by substance misuse and dependence.
- Provide high-level communication skills to engage and collaborate with consumers, their families and carers, and other stakeholders.
- Utilize a harm minimization approach when intervening to support consumers affected by substance misuse and dependence.
- Collaborate with multidisciplinary teams to provide comprehensive care and support services.
- Develop and maintain strong relationships with community partners and stakeholders to ensure seamless service delivery.
- Tertiary qualifications and current registration with AHPRA for Registered Nurses, Occupational Therapists, Psychologists, or eligibility for membership of the Australian Association of Social Workers for Social Workers.
- Demonstrated understanding of and skills in evidence-based interventions for substance use and dependence.
- High-level communication and interpersonal skills, with the ability to engage and collaborate with consumers, their families and carers, and other stakeholders.
- Ability to work independently and as part of a multidisciplinary team.
- Current unrestricted drivers' license and the ability to maintain.
