Job Title: Registered Nurse

Objective:

We are seeking a skilled and compassionate Registered Nurse to join our Home Care team. In this role, you will provide high-quality clinical care and support services to participants in their homes.

  1. Provide comprehensive nursing assessments, develop care plans, and implement evidence-based interventions to enhance the health and well-being of participants.
  2. Collaborate with multidisciplinary teams to coordinate care, address participant concerns, and facilitate smooth transitions between care settings.
  3. Conduct regular home visits, monitor participants' conditions, and report any changes or concerns to the supervisor.

Requirements:

  • Current registration as a Registered Nurse with the relevant state nursing board.
  • Experience working in community or home care settings.
  • Strong communication and interpersonal skills to work effectively with participants, families, and healthcare professionals.
  • Ability to prioritize tasks, manage multiple responsibilities, and maintain accurate records.
