About this Role
  • Perform in-home clinical assessments, reviews, and care needs for clients.
  • Provide clinical advice and support to clients, referring them to relevant services when necessary.
  • Collaborate with local allied health professionals to ensure responsive care delivery.
  • Engage in health education and promotion activities, including social support groups and carer education.
  • Promote a harmonious team approach and implement safe practice measures.
Requirements and Qualifications
  • A Bachelor's degree in Nursing or equivalent qualification accompanied by current AHPRA Division 1 registration.
  • A minimum of 3 years' experience as a Registered Nurse in Community, Commonwealth Home Support or District Nursing.
  • Demonstrated teamwork skills, positive outlook, and flexibility in approach.
  • Endorsement for Continence Assessments and Prescription (desirable).
  • Experience with evaluating and implementing care support plans.
  • Familiarity with the My Aged Care portal.
  • Proficiency in decision-making, critical analysis, and problem-solving.

This is a full-time position, Monday to Friday during business hours. The office-based role offers flexibility in accommodating organization and client needs.
