About the Role

We are seeking a skilled Dietary Specialist to join our team at Lifecare Homecare. As a community Dietitian, you will work with clients to provide nutrition assessment and planning of dietary needs, including food service management and intervention programs to enhance health and wellbeing.

Key Responsibilities
  • Deliver high-quality Dietetic services tailored to individual client needs, using a person-centred approach.
  • Conduct assessments and provide interventions to patients and clients in their own homes.
  • Collaborate with Allied Health Professionals, GPs, and other Healthcare Professionals to provide effective patient care.
  • Write reports that meet the standards of funding bodies, such as NDIS, DVA, Medicare, and HCP Providers.
  • Reflect our practice values of Care, Respect, Integrity, and Service Excellence in all aspects of the role.
  • Maintain effective relationships with Case Managers, GPs, community centres, and other referrers.

Requirements
  • Bachelor of Nutrition and Dietetics or equivalent that meets the requirements for full or part-time membership of Dietitians Australia.
  • New graduates welcome to apply.
  • Ability to work within a multi-disciplinary team and autonomously in the community.
  • An enjoyment of working with older adults and adult disability demographics.
  • Full working rights in Australia.
  • Clean driver's licence, access to own vehicle, and comprehensive car insurance.
  • Current Working with Children Check and NDIS Workers Screening Clearance, or ability to obtain.
  • Willingness to undergo a National Criminal History check upon successful application.
