**Job Title:** Home Health Care Nurse
About the Role:We are seeking an experienced Home Health Care Nurse to join our team. As a key member of our community health services, you will provide high-quality medical care to children, youth, adults, and seniors in their homes and other settings across Nova Scotia.
You will be responsible for assessing clients' needs, developing personalized care plans, and implementing interventions to promote optimal health outcomes. Your expertise in diabetes management, dialysis, medication management, palliative care, phlebotomy, and wound care will make a significant difference in the lives of our clients.
Key Responsibilities:- Assess clients' physical and emotional needs, and develop comprehensive care plans
- Provide direct patient care, including administering medications, performing phlebotomy, and providing wound care
- Collaborate with healthcare teams to ensure seamless care coordination
- Maintain accurate records and reports, and participate in quality improvement initiatives
- Licensed as a Licensed Practical Nurse (LPN) in the province of practice
- Experience in community health, preferably in home health care or similar settings
- A valid driver's license and access to a reliable vehicle
- A current and original copy of a satisfactory Criminal Records Check, and a Vulnerable Sector Search (if required)
