Salary: $47.66 per hour + superannuation
Job Description:
We are seeking an experienced Aged Care Assessor to join our dynamic team. As an Aged Care Assessor, you will be responsible for conducting assessments to identify and meet the needs of clients, referring them to appropriate home support services.
Your Key Responsibilities:
- Collaborate with clients, families, and multidisciplinary teams to determine client needs and develop support plans.
- Conduct accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client eligibility for home support services.
- Review support plans to ensure they meet changing client needs and provide ongoing support.
- Work effectively both autonomously and as part of a broader team to deliver key performance indicators within identified timeframes.
- Maintain currency of knowledge relating to aged care services, both funded and non-funded.
- Assist with continuous improvement frameworks to evaluate and enhance assessment processes and outcomes.
Required Skills and Qualifications:
- Australian Driver's license and access to a registered, insured vehicle.
- Excellent communication skills, both written and verbal.
- Commitment to excellence in customer service, both to internal and external stakeholders.
- Excellent planning, organizing, and problem-solving skills.
- Previous knowledge or work experience in the Aged Care System preferred.
- Completion of training requirements as specified under the My Aged Care workforce learning strategy 2023.
- Demonstrated understanding of person-centred, goal-directed, strength-based holistic assessments.
- Demonstrated understanding of work health and safety principles, including risk management.
- Willingness to apply for a Police Check.
