">About the RoleThe Aftercare Lead is a strategic leadership position responsible for integrating aftercare services across the district.
This includes leading the coordination of aftercare services with hospital and community mental health and suicide prevention services to ensure access to support for all who need it.
To achieve this goal, you will collaborate with senior health managers, clinical leaders, research partners, and external stakeholders to drive project management and change management initiatives.
This role requires strong analytical skills, effective communication, and the ability to interpret complex information rapidly and accurately.
As a valued member of our team, you will contribute to enabling our community to be healthy and well, providing compassionate care when needed.
">Key Responsibilities- Lead the integration of aftercare services across the district
- Collaborate with stakeholders to drive project management and change management initiatives
- Analyze complex information to inform decision-making
- Develop and implement strategies to improve access to aftercare support

">RequirementsTo be successful in this role, you will require:
- Tertiary qualifications and/or equivalent work experience relevant to mental health and suicide prevention
